What is the Herpes Simplex Virus (HSV)?
The herpes simplex virus (HSV) is a common viral infection that affects the skin and mucous membranes. It is divided into two types: HSV-1 and HSV-2. Both types can cause different lesions in various parts of the body.
HSV is most commonly seen as oral herpes (around the mouth) or genital herpes (in the genital area). These infections often lead to painful blisters or ulcers, with genital herpes being especially uncomfortable. The most typical symptom is fluid-filled blisters that eventually open and crust over, also known as herpes outbreaks. However, HSV can also infect individuals without causing visible symptoms.
The severity of herpes depends on the virus type and the person’s general health. While there is no permanent cure, available treatments help reduce outbreaks, make symptoms more manageable, and lower the risk of transmission.
Types of Herpes
HSV-1
HSV-1 is the most common type of herpes simplex virus and often appears early in life. While many people carry it without symptoms, when active, it usually causes cold sores around the mouth. HSV-1 is the primary cause of oral herpes but may also lead to genital herpes through oral sexual contact.
HSV-2
HSV-2 is less common but is the main cause of genital herpes. Transmission typically occurs through sexual activity.
Both HSV-1 and HSV-2 spread through direct skin contact or body fluids and can remain dormant in the body for life. Even when no symptoms are visible, the virus can still reactivate and spread.
Is Herpes Contagious?
Yes, herpes simplex virus is highly contagious. It spreads through close skin contact, including kissing or sexual contact. Once infected, the virus stays in the body for life. Although it often remains dormant, reactivation can cause outbreaks and increase the risk of transmission.
Incubation Period of HSV
The incubation period of HSV is generally 6–8 days but can range from 1 to 26 days. Some people show symptoms soon after infection, while others may remain symptom-free for months or years, with the virus staying dormant until it reactivates.
Causes of Herpes
Herpes spreads through infected oral or genital secretions. Common transmission routes include:

Symptoms of Herpes

General symptoms: Fever, headaches, swollen lymph nodes, eye irritation, and light sensitivity
Areas Affected by HSV
HSV most commonly affects the mouth and genitals but can also appear elsewhere:
Mouth & face: Cold sores around lips, sometimes inside the mouth or nose
Genital area: Painful sores on visible and hidden areas such as the vulva, penis, or cervix
Eyes: Can cause herpes keratitis, a serious eye infection
Nervous system: Rarely, HSV may cause encephalitis or meningitis
Other areas: Fingers (herpetic whitlow), skin (herpes gladiatorum), and in severe cases, internal organs like lungs, liver, or esophagus
Diagnosis of Herpes
A physical examination is often enough to diagnose herpes, especially if blisters are visible. In some cases, blood tests may be used to detect antibodies against HSV-1 or HSV-2. If more serious conditions such as encephalitis or meningitis are suspected, additional tests like a spinal tap may be performed.
Treatment of Herpes
Although herpes cannot be permanently cured, antiviral medications help control outbreaks and reduce symptoms. Treatments may include:
Antiviral pills
Creams or ointments
Intravenous medication in severe cases
Eye drops for herpes-related eye infections
Types of Treatment Approaches
Episodic therapy: Short-term antiviral treatment during outbreaks
